Levels of Care

Two lanes of care, one front door.

Mental health services and substance use services are provided under separate authorities, so each has its own levels of care. One call finds the right starting point — and if it turns out we are not it, we say so.

Detox, outpatient, intensive outpatient, and medication-assisted treatment are provided under California DHCS substance use disorder certification 370254AP. Partial hospitalization is offered only as a specialty mental health service under the county contract.

If we are not the right place, we will tell you where is.

We are outpatient only — no residential beds, no inpatient detox. When the assessment says you need a level of care we do not provide, we say so on that call and help you reach the County's Access and Crisis Line or a provider closer to you.

Housing support, built into treatment

2×

the odds of staying sober

In a randomized trial in the American Journal of Public Health, people who moved into structured recovery housing after treatment were about twice as likely to still be sober two years later — 69% vs 35% under usual aftercare.

Mental health moves the same way: in pooled randomized trials of homeless adults, most living with mental illness, housing meant significantly fewer hospitalizations and ER visits — and UCSF's statewide study found 44% of unhoused Californians with a psychiatric hospitalization were first hospitalized only after losing their housing.

No stable housing? We help plan it with your treatment.

Many of the adults we treat — for mental health, substance use, or both — are unhoused or in an unstable living situation. That is why housing is part of intake here:

  • Sober living partners throughout San Diego County
  • 140+ recuperative care beds through our partner network
  • Housing coordination for mental health PHP/IOP clients, not just addiction treatment
  • No charge to Medi-Cal clients — covered by carve-outs, never billed to you

Your Care Team

The people you will actually see.

A small team, in one building, treating adults across both lanes of care — psychiatric, clinical, and the practical work of getting you covered and enrolled.

01

Psychiatric care

Medical Director · board-certified psychiatrist

Dr. Sanjai Mathew Thankachen provides clinical oversight of psychiatric care, medication management, and the medical side of co-occurring treatment, with an emphasis on patient safety and evidence-based practice.

02

Licensed therapy

Program Director, LMFT · licensed clinicians

Matthew Beck, a licensed Marriage and Family Therapist with more than a decade in substance use and mental health treatment, oversees clinical delivery and holds the standard of care across both programs.

Coverage & coordination

Admissions · Medi-Cal and county billing

Our admissions team verifies your benefits before you commit to anything, coordinates care across the mental health and substance use lanes, and refers out honestly when the assessment points somewhere else.

Both Lanes, One Roof

When a mental health condition and substance use show up together.

Most places treat one and refer out the other. Golden Coast holds both authorities — county-contracted specialty mental health and DHCS-certified substance use care — so the two can be treated in the same building by the same team.

Starting from mental health

Co-Occurring Disorders

A mental health condition treated as the primary diagnosis, with the substance use addressed alongside it rather than deferred until after.

Dual Diagnosis

You came in for alcohol, opioids, or stimulants — and the anxiety, depression, or trauma underneath gets treated at the same time, not sent elsewhere.

— Admissions

Starting here takes one call, not an application.

There is no form to wait on and no cost to find out whether we are the right fit. Four steps, most of them finished the same week.

  1. The first call

A confidential conversation about what is going on. Have your Medi-Cal or insurance card nearby if you can find it; if you cannot, your name and date of birth are usually enough to start.

  2. Coverage check

We verify your benefits before you commit to anything. With Medi-Cal in San Diego County — any of the four plans — treatment here can be covered at no cost to you.

  3. Clinical assessment

A clinician talks with you about symptoms, history, substance use, medications, and safety, then recommends a level of care. This is also where we tell you if we are not the right place.

  4. Start date

If we are a fit, we schedule your first day, walk you through the schedule, and tell you exactly what to bring. Most people start within days rather than weeks.
